3. Enhancing your confidence and skills to proficiently use and coordinate your supports.
Support coordination services are typically delivered by a support coordinator or specialist support coordinator.
Your plan may include one of the following three levels of support coordination:
1. Support Connection: This level aims to improve your ability to connect with informal, community, and funded supports, empowering you to maximize your plan and pursue your goals.
2. Support Coordination – Coordination of Supports: This support level helps you develop the skills necessary to understand and implement your plan. Support coordinators like Ms. Thompson and Mr. Thompson collaborate with you to ensure a diverse range of supports is employed, enhancing your capacity to maintain relationships, manage service delivery tasks, live more independently, and participate in your community.
3. Specialist Support Coordination: Geared towards individuals with more complex situations requiring specialized assistance, this higher level of support coordination helps manage challenges in your support environment and ensures consistent service delivery.
Your plan's funding is based on what is deemed 'reasonable and necessary' for goal pursuit, complemented by support from family, friends, and community or government services. Support coordination is included in the Capacity Building budget, providing a fixed amount for a support coordinator, such as Ms. Thompson or Mr. Thompson, to assist you in implementing your plan. If your plan specifies the funded level of support coordination, you can only acquire that particular level. Alternatively, if your plan doesn't specify, you have the flexibility to choose the support coordination level that aligns with your needs.
